LONGVIEW, Texas The EverStart Series will visit Lake of the Pines Oct. 4-6 for the fourth and final stop of the Texas Division. The tournament will feature as many as 300 pros and co-anglers competing for top awards up to $35,000 plus a Ranger Z518 boat with an Evinrude or Mercury outboard for the pro and a Ranger Z117 boat with an Evinrude or Mercury outboard plus $5,000 for the co-angler.

?This lake is so good anybody could win,? said Kellogg?s Rice Krispie Treats pro Jim Tutt of Longview, Texas. ?No one will have an advantage. I think it is one of the top bass-fishing lakes in the country. The lake is full of grass, lily pads, shallow flats . . . it has everything.

?The lake is divided in two,? Tutt continued. ?The north end has shallow flats, trees and lily pads; the south end is like a low inland reservoir with lots of points, river channels and timber. Anglers will be able to catch bass in a multitude of ways ? topwater, frogs, buzzbaits, deep-diving crankbaits and Senkos. It?s really like a mini Toledo Bend.

?The lake has been fishing tough this summer because it has been super hot, however, it is starting to cool down and things are changing. I really hate to guess at what winning weights are going to be. If the lake is on, it is going to take 25 pounds per day to win. If it?s not, probably more like 17 pounds per day. I will tell you one thing. There is a good chance a 10-pounder will be caught at this tournament.?

Tutt said that the lake is about 4 ?-feet low, which will make the lake fish small. However, there are plenty of bass to be caught. According to Tutt, the key to winning the tournament is going to be finding the right school of fish and sticking with it. ?Once you find that school, you should stay put. Don?t go running around the lake,? Tutt said.

Anglers will take off from Lakeside Area Recreation Area located at 2669 FM 726 in Jefferson, Texas, at 7 a.m. daily. Thursday and Friday?s weigh-ins will also be held at Brushy Creek Recreation Area beginning at 3 p.m. Saturday?s final weigh-in will be held at the Walmart located at 515 E. Loop 281 in Longview starting at 4 p.m. Takeoffs and weigh-ins are free and open to the public.

Pros will fish for a top award of $35,000 plus a Ranger Z518 boat with a 200-horsepower Evinrude or Mercury outboard if Ranger Cup guidelines are met. Co-anglers will cast for a top award consisting of a Ranger Z117 with 90-horsepower Evinrude or Mercury outboard and $5,000 if Ranger Cup guidelines are met.

The EverStart Series consists of five divisions ? Central, Northern, Southeast, Texas and Western. Each division consists of four tournaments and competitors will be vying for valuable points in each division that could earn them the Strike King Angler of the Year title along with $5,000 for the pro and $2,000 for the co-angler. The top 40 pros and co-anglers from each respective division will qualify for the EverStart Series Championship that will be held on the Ouachita River in Monroe, La., Nov. 1-4.

The EverStart Series tournament on Lake of the Pines is being hosted by the Longview Convention Visitors Bureau.

Connecticut man shoots burglar dead, turns out to be own son

(Reuters) - A Connecticut man responding to his sister's call for help during an apparent burglary at her home next door, shot and killed a masked intruder who turned out to be his own teenage son, state police said on Friday.

Tyler Giuliano, 15, was wearing a ski mask and appeared to be armed when he was shot on Thursday by his father, who authorities declined to identify, said Lieutenant J. Paul Vance, a spokesman for the Connecticut State Police.

The father's sister, who lives next door, was home alone before 1 a.m. when she called him to report someone trying to break into her home. The father went over to investigate and was approached by a masked person dressed entirely in black and holding a shiny object, police said in a statement.

"Believing the suspect was armed with a weapon and about to attack him, the (father) discharged his personal handgun at the suspect," police said in the statement.

Giuliano was pronounced dead at the scene.

"(He) was lying on the ground in the driveway with obvious gunshot injuries, holding a weapon," the statement said.

Vance declined to further describe the weapon.

Authorities seized the father's gun, were investigating whether it is registered and will consult with the state's attorney to determine if any charges will be filed, Vance said.

Police also were investigating why the teen, who has no criminal history, was outside his aunt's home.

"We truly don't know. We'll look at the family dynamics, (his) school locker, cell phone, computer to figure out what's going on," Vance said.

Web Marketing Ideas To Build Your Business Or Brand ...

It can be hard to do website marketing if you?re just beginning, especially when there is so much competition in your field. The best way to use Internet promotion is by using many techniques. This article will help you get started!

TIP! Be sure to provide an ordering system that is secure. Make sure that you are using a safe website when placing any type of money transaction option on your website.

Advertising is necessary to sell anything, whether online or not. Although the up-front expenses can be high, the traffic and leads that ads generate make the investment worthwhile. Advertising can give you access to customers you might not find in any other manner.

The best part of a solid marketing strategy is having solid content. Your content should be checked frequently to ensure that its quality remains high and that the message it sends to customers is consistent. In addition, make sure that someone looks over your content. A fresh perspective is extremely valuable. Finally, you must be sure that you run a spelling and grammar check on your content to ensure everything is correct.

TIP! You should invest time to learn how to design websites. There is a wealth of information about HTML and CSS online, or you could check out a book at the local library.

Offer customized versions of your website to people in other countries. You can do this by implementing a version of your site in several different languages. This will result in a larger, global audience. Customers will be far more likely to purchase your products and services if you offer the information in their own languages.

Look at Internet promotion as something fun and not something that is a job or big chore. Do this for fun until the time comes when you know you can actually make a career out of it.

TIP! Attach a human face to the brand. Use your real name in your ads and post a picture of yourself on your website.

With any kind of marketing, you are going to run into setbacks. The best thing to do is deal with them and keep on going. When you are planning a product or service launch, you must make sure you have done all your research. This may require a significant initial work investment. Keep your focus on your end result and realize that your hard work will pay off.

Try placing banners where they don?t look like banners. If they appear as clickable links, your customers will be more apt to use them. Many people won?t click banners if they look like banners, so this can entice them.

TIP! Provide a safe and secure ordering and payment process for your Internet business that is easy for your customers to understand. People are hesitant to send out personal information via the internet so do your best to assure them that your site is secure.

A great way to increase traffic to your site is to offer a free course. 2 G4S officers resign after Olympics debacle

(AP) ? Two leading officials at G4S have quit following the company's poor handling of the Olympics and Paralympics security contract, but chief executive Nick Buckles has held onto his job.

In a statement Friday, G4S said Chief Operating Officer David Taylor-Smith and Ian Horseman Sewell, managing director of global events, have resigned for the debacle which cost the company both in terms of money and reputation. In the run-up to the Olympics, which started in late July, the British Army was called in to plug gaping holes in G4S's plans.

Though G4S said the CEO has "ultimate responsibility" for the company's performance, it said a review of the debacle did not uncover "significant shortcomings" in Buckles' performance or in his handling of the Olympics contract.

The board's review said the company failed to recognize the challenges the Olympic contract would impose because of its scope, and added that ineffective monitoring and tracking of the security workforce led to problems.

Chairman John Connolly said the company will learn from its mistakes and strengthen its management and governance procedures. The company also announced a series of changes designed to prevent future problems.

New ad seeks to show Romney's compassionate side

Republican presidential candidate, former Massachusetts Gov. Mitt Romney campaigns at American Legion Post 176 in Springfield, Va., Thursday, Sept. 27, 2012. (AP Photo/Charles Dharapak)

(AP) ? TITLE: "Too Many Americans"

LENGTH: 60 seconds.

AIRING: Mitt Romney's campaign did not disclose where the ad is running.

KEY IMAGES: Romney, wearing a check-pattern shirt open at the collar, speaks into the camera. "Too many Americans are struggling to find work in today's economy. Too many of those who are working are living paycheck to paycheck, trying to make falling incomes meet rising prices for food and gas."

He says more people are living in poverty than when President Barack Obama took office and that 15 million more are on food stamps.

"President Obama and I both care about poor and middle-class families," Romney says. "The difference is my policies will make things better for them. We shouldn't measure compassion by how many people are on welfare. We should measure compassion by how many people are able to get off welfare and get a good-paying job."

Romney adds that his plan will create 12 million new jobs over four years to help families escape poverty and strengthen the middle class.

ANALYSIS: Romney is trying to convince voters that he can do a better job fixing the economy than Obama but, beyond claiming that his plan would create 12 million jobs, he doesn't say anything in the ad about what his plan entails.

The ad is an attempt to show a more compassionate side of Romney after his campaign was stung by the release of secretly recorded video of Romney speaking at a private fundraiser in May and disparaging the "47 percent" of Americans who don't pay federal income taxes as government-dependent Obama supporters who see themselves as victims and won't take responsibility for their own lives.

Public opinion polls conducted after the "47 percent" was made public last week give Obama the advantage in battleground states, including Ohio and Virginia.

The ad tries to cast Romney as someone who understands the challenges faced by families struggling to make ends meet under trying economic times. The sluggish economy has long been front and center in the presidential contest. Democrats have sought to portray Romney as a wealthy former venture capitalist who cannot relate to the worries and economic woes of average folks.

Romney's campaign has tried on several fronts in recent weeks to warm up the former Massachusetts governor's image for voters, including a speech by Romney's wife, Ann, at the Republican National Convention that focused heavily on Romney's personal qualities, including his compassion.

Obama is running an ad in seven battleground states that is somewhat similar in style to Romney's. In that ad, the president also speaks into the camera and promotes his economic plan and a "new economic patriotism."

This type of straightforward candidate-as-narrator ad is more commonly used during the closing days of major races. Both of these ads come as early voting gets under way in some states.

Dana White at Jones' defence once again ? Boxing News | bettor.com

Dana White at Jones? defence once again ? UFC News

Finally, it seems like the feud between president of the Ultimate Fighting Championship Dana White and light heavyweight champion Jon Jones has been settled. After watching Jones put up everything at stake against Vitor Belfort at UFC 152, White believes Jones should be credited for his accomplishments.

?There's no doubt that this was Jones' best performance. He looked phenomenal tonight,? said White during the post-fight press conference.

Jones faced the toughest fight of his life against Belfort. He was locked in an armbar submission right in the first round by the Brazilian fighter yet Jones did not give up. During the press conference, Jones admitted that he felt his arm was about to break but he was not ready to give up. He claimed that he would have carried on even if he had to fight with one arm.

When asked about the injury, Jones said that after the submission hold, he felt like his arm was not in the right condition yet he used it to hit Belfort because he is trained to do so.

White and Jones had their bitter moments following UFC 151?s declination recently. However, White believes everyone makes mistakes in their young age and so did Jones. White is ready to forgive the champion for his wrongdoings.

?At the end of the day, has he made a few mistakes? Of course he has. He's not a bad guy. He's not a bad kid at all. He and I sat down yesterday, we had a great conversation...Everything is awesome.?

?There can be no more naysaying about Jon Jones. Listen, you love him or you hate him. If you don't like him, whatever, if you like him, good, but you can't deny that the kid is extremely talented and one of the best in the world...I disagree, and I think that Jon Jones did look unbeatable tonight.?

As of now, White is once again impressed by the American champion, believing that it is about time for naysayers to start respecting Jones. White also highlighted the fact that Jones was highly criticised for not being able to take punches, but now he did and he has proved himself as a top-notch fighter.

Alternative And Wind Power Reviews: Energy Auditor: The Best ...

Advanced technology and development of machinery supports us with improved methods to help us with our daily activities. As a consequence, the trend of energy and electricity consumption is rising, whether in industrial, office or home applications. The fact that electricity becomes one of the significant components in today's industry opens a rather new perspective. To make sure the electricity and equipment is optimally used, homes and institutions need the help from an energy audit.

Energy Auditor Job Description

An energy auditor is responsible for conducting investigation to measure the degree of cost-effective uses of energy. The auditor's job description includes among other things analyzing energy bills to collect the usage data, calculating potential for electricity saving, performing tests such as blower door tests to detect air leaks, educating and recommending to customers more energy-efficient appliances or alternatives to the currently used source of electricity, and checking the installations of pipe insulation, water heater wraps, door sweeps, low flow showerheads, and weather-stripping for enhancing energy efficiency.

Types of Energy Audits

Generally, energy audits can be divided into two groups, home and commercial energy audits. Home audits are focused on the inspection of a home's electricity efficiency by using professional equipment such as infrared cameras and blower doors. The evaluation prioritizes the improvement of energy efficiency in heating and cooling the house. This method may include the recording of certain energy transmission characteristics in a home including the walls, floors, ceilings, doors, skylights and windows. Additionally, it may also evaluate the mechanical system in the house including the ventilation, air conditioning equipment, thermostat and heater. In the United States, such service can be conducted by the US state energy office, private-independent energy auditors or companies, or public utility companies.

Although commercial energy audits more or less provide similar services to the home type, the skill set used is more advanced. While home audits are concerned more with weatherproofing and home insulation, the industrial one focuses on facilities that contribute to the largest energy consumption such as lighting, production equipment and HVAC.

Education and Skills Required

At present, there are no specific majors in colleges that provide the education required to become an energy auditor. However, one who seriously considers pursuing the job as a career choice can take energy audit training and certification programs. During the training you will be taught some skills and knowledge about science, buildings, diagnosing air leakage, building assessment skills, durable healthy homes, heating and cooling systems, building envelopes, water heating and base load, thermal imaging, and energy audit software.

To gain a good grasp on the training materials, it is better if you equip yourself with basic skills such as reading comprehension, active listening, speaking, writing, critical thinking, time management, operation analysis, service orientation, and quality control analysis. On the other hand, you will also need to master the knowledge of building construction, mathematics, mechanics, physics, engineering and technology, sales and marketing, administration and management, computers and electronics, and design.

Energy Audit Oversight

Based on data gathered from the Bureau of Labor Statistics of the 2010 wage data and 2008-2018 employment projections, until currently there are about 994,000 energy auditors employed in the United States alone. The salary for the job averages $67,710 per year or about $32.55 per hour.

Energy audit is necessary for helping you save energy consumption. Learn more about this job or find energy auditors by visiting our site. Providing articles, reviews and writings on energy, alternative energy and other environmental issues online.

Buying a Racehorse

Due to the high demand by consumers, there are many racehorses for sale. Before buying a racehorse, you will have to first set your budget. The price of a racehorse depends on its age, its quality and also the location. And older horse is usually more expensive than a young foal. But as it ages, its price drops. There are many breeds of horses, some which are rare. Thus, certain breed or quality of horses will be more expensive compared to others. The price of horses also depends on the place. Generally speaking, a horse in Asia will be much cheaper that a horse in America.
Racehorses for sale come in many methods. If you have a tight budget, you can buy shares in racehorses instead of the animal itself. By buying shares in racehorses, you will be entitled for a portion of the horse race winnings throughout a season or more - depending on the contract. Buying shares in racehorses is also a good option for people who do not know how to care for horses. Buying a share will include hiring a caretaker to care for the horse on the behalf of its owners. This amount might also include training expenses to help your horse train for coming races and competitions.
If you have the money and the expertise for caring and training a racehorse, you can look out for thoroughbreds for sale. Thoroughbreds are racehorses that are agile, fast and obedient. These horses are especially used in equestrian and racing events. With proper training and care, your racehorse might be the next big star in horse betting or in equestrian competition. Buying thoroughbreds is a form of investment for those who are capable. But always remember that there will be risks involved when you buy and train your own horse.
Looking for a racehorse dealer in person might be quite difficult as there are not many people who are trained in this field. And in most cases, owners will not be willing to sell horses that they have cared and trained for many years. If you are looking for racehorses for sale, you can try looking on the Internet. There are many websites which offer race horses for sale and buying shares in racehorses. Do your research and look for a horse that is fitting for you to invest in. You should search several websites to find a racehorse that is cheap, yet of high potential. Do not buy a horse that is already at its prime as its value will depreciate. And if it is needed, visit the horse for a physical inspection and take it for a ride before you make the purchase. Buying a racehorse is not a simple matter as horses are expensive and require extensive care.

Keku Assigns Local Numbers to Your International Friends for Super-Cheap CallsKeku Assigns Local Numbers to Your International Friends for Super-Cheap Calls iOS/Android: If you do any international calling, you know it can cost a lot of money. There are ways around it, but Keku is a service that assigns your friends abroad numbers that are local to you, so you can call them affordably, even if they're not using the same app. If they are, your calls are completely free.

Keku supports "local" calling in 41 countries, and the service works through its iOS and Android apps. When you call your international friends through Keku, you still have to pay for it (unless they're also using Keku, so it makes sense to try and get them to sign up), but the rates are generally cheaper than your carrier's. To compare rates between the countries you call, check Keku's rates page here.

The service also offers affordable group calling (iOS only, unfortunately), where you just select the participants and Keku handles connecting them all into a conference for you. To give the app a try, grab the app from from the iTunes App Store or Google Play.

Insight: China housing drive gives measure of next Premier

MENGTOUGOU, China (Reuters) - One day last summer, the man slated to become China's next premier strolled through a run-down alley between cramped brick huts and then toured a construction site a few miles away where brightly painted tower blocks promised new homes for the country's poor.

The contrasting neighborhoods, both in Beijing's poor, mining region of Mengtougou, embody the affordable housing policy overseen by Vice Premier Li Keqiang, who is expected to replace Premier Wen Jiabao in an upcoming leadership handover.

Affordable housing - and how to pay for it - will be one of the defining challenges of the next leadership as it tries to deal with the high housing prices that are one of the greatest sources of discontent in today's China.

Li's personal involvement hasn't resolved all of the problems of a program fundamental to the Communist Party's claim on power, but it gives some insight into the broader issues he would have to deal with as premier.

Chinese leaders generally keep their cards close to their chest, but Li's long association with affordable housing suggests a populist leadership style even if his policy preferences remain unknown.

His visits to Mengtougou are helping create an image of a politician in the mode of current Premier Wen, who is often portrayed in media as shedding tears over the plight of China's poor.

"Li has been trying to establish himself as a Wen Jiabao-like populist who looks after the disadvantaged," said David Kelly, who follows elite politics closely for Beijing-based consultancy, China Policy.

Li says he cried on his first visit to a shanty town in frigid Liaoning Province in northeast China eight years ago -- a moment that state media now evokes to show his long association with the cause.

During that 2004 visit as a newly-appointed party secretary in Northeast China, Li promised to help laid-off miners in the blighted city of Fushun. Within a year, 13 new residential housing blocks sprang up to replace the ramshackle slum - an initiative that is now portrayed as a defining moment in China's affordable housing policy.

"This was Li Keqiang's contribution to China's economic development, and to the development of society," said Meng Xiaosu, a former classmate of Li's, who helped draft China's housing reforms in the 1990s.

"He saw the workers in impoverished housing conditions. It was just like the Industrial Revolution-era England that we just saw during the London Olympics opening ceremony."

MAKING HOUSING AFFORDABLE

While the local issue in 2004 was basic housing in an economically depressed area, the national policy now aims to ease discontent over the high price of buying a home.

Average urban disposable income is just 21,810 yuan ($3,500) a year, but prime apartments sell for millions of yuan.

The program being championed by Li aims to build 36 million units between 2010 and 2015, at an estimated cost of $800 billion. Through August, China had built just over 4 million such homes in 2012 against a full-year target of 5 million.

However, critics say the program is a mish-mash of initiatives, uneven building standards and poor planning, issues often highlighted in Chinese media reports.

Targets are missed, scrapped and re-set, leaving pledges to the poor in places like Mengtougou -- a hilly county pocked with depleted coal mines -- ringing hollow.

Some of these issues, such as missed targets, were apparent during a visit to the area by Reuters. Other criticisms, such as shoddy construction and how well-connected people are able to jump to the top of the waiting list, were not in evidence.

The 144-tower complex Li visited last year is designed to house 40,000 people resettled from Mengtougou's mining villages and squatter towns. Today, construction is still underway at most blocks.

New residents complained that local authorities razed their former homes to meet relocation quotas and free up land long before their new units were completed. That left families to bounce between expensive temporary rentals for as long as three years.

Children have to commute for miles to their former schools, hospitals aren't built yet and extended families crowd into tiny rooms, awaiting the completion of apartments they were promised.

Finished buildings are brightly painted and roses bloom in the courtyards. For many, the new apartments provide a higher living standard than their former rooms, which lacked heating or plumbing.

"The biggest improvement is that I do not have to go to a public toilet in cold winter nights and that I do not have to cook with coal," said 80-year-old Zhou Shu, proudly showing off the gas cooker in her new 40-square-metre apartment.

Under the affordable housing program, Beijing gives construction targets to regional authorities each year.

To meet targets, some local authorities simply reclassify homes built for government employees or state-owned enterprises as part of the affordable housing program.

A Reuters visit to Shijiazhuang, a city southwest of Beijing, found that a number of apartment units labeled "affordable housing" were actually factory or teachers' dormitories.

In other cases, developers secured permits for luxury housing projects by including a few "affordable" units.

A middle-income city of 10 million people, including satellite towns, Shijiazhuang is one of the few to have published a public list of its affordable housing projects.

WEALTH GAP

Few topics get Chinese as riled up as property. The tension is particularly acute since many have gotten rich by flipping property in the 15 years since housing reform first began.

During Wen's decade as premier, average home prices rocketed 10-fold, dramatically widening a wealth gap the Party is ideologically impelled to close.

Meanwhile, the 230 million migrants looking for work in densely crowded cities cram into factory compounds or in dingy dwellings lacking basic facilities like indoor plumbing. Others stay in dormitories on construction sites, ironically building luxury apartments that speculators never live in.

Wen promised to bring housing costs down to an undefined "reasonable level" - a move many economists see as a throw-back to the era of top-down planning controls. His lending restraints have weighed on economic growth, setting it on course to hit a 13-year low this year, but have done little to dent property prices.

Beijing refuses to relax the property curbs, in place for nearly three years, for fear of unleashing a sudden price surge, even as it tries other measures to lift economic growth.

Property investment generates about half of the country's economic output and directly affects 40 industries.

"Li has been behind the (affordable housing) program so when he becomes premier there will be a strong mandate to continue," said Arthur Kroeber, managing director of economics consultancy GK Dragonomics in Beijing.

"The question is not the trajectory of the policy but the question of how they will pay for it."

The longer the property curbs remain, the more they drag on not only economic growth, but also the government's plans for subsidized housing.

Cities rely on land sales for most of their revenue. Unable to sell land to property developers because of the curbs, many are running short of funds for other obligations, including paying for the low-income homes in the affordable housing program.

Li's former classmate Meng, who helped draft China housing reforms, advocates freeing up private housing construction to generate land revenues to fund the housing targets.

Li himself has so far sidestepped the question of how the government should define and maintain the "right" property price. He has never publicly aired his own views on the broader curbs, in line with China's tradition of political consensus.

Indeed, some analysts suggest housing is too hot an issue to handle and Li may decide to step back from personally overseeing the program once he is premier.

"As premier he can set priorities. If I were premier, I would choose some easy targets. The housing market isn't easy," said Bo Zhiyue, a political analyst at the East Asian Institute of the National University of Singapore.

"Do you want to raise the price or lower it? Do you want it to crash?"

EARLY YEARS

Li himself is no stranger to hardship or upheaval.

As an 18-year old in March 1974 - two years before the end of the Cultural Revolution - Li was one of tens of thousands forced by Mao Zedong's "sent-down youth" campaign to drop school work, toil on farms and learn from the peasantry.

He was assigned to a work brigade in rice-growing Fengyang County, in eastern China's Anhui Province, where he joined the Communist Party. Over a fifth of Fengyang's population had starved to death 15 years earlier in the disastrous collectivization drive of Mao's Great Leap Forward.

In the late 1970s, shortly after Li left Fengyang, starving farmers started to break free of communist collectives, inspiring Deng Xiaoping's reform to allow farmers to plant their own land.

Chen Xueqin, 70, who lived across a dirt path from Li in Fengyang nearly 40 years ago, remembers his singular focus, while fellow villager Xu Weifang, 78, recalls an introvert who was always reading.

"When he talked it was about work," Chen said.

Today, Li appears more at ease in small groups than in public. Businessmen and academics say they have been impressed with his diligent studies of policy.

A law student at prestigious Peking University in the 1980s, Li was caught up in the fervor of political and economic reform. He helped translate a British book, "The Due Process of Law", into Chinese.

He stayed well within the bounds of Party policy and priorities, even while advocating in his master's thesis mildly reformist ideas for moving manufacturing to country towns and loosening China's restrictive residency system.

Traces of his concern for law and process could be seen in late August, when he began a policy meeting for allocating affordable housing by visiting a neighborhood committee and emphasizing fairness in assigning apartments.

The question for investors and analysts, though, is whether affordable housing demonstrates Li's effectiveness as a policymaker or potential as a game changer in China's consensus-based system.

Li's association with affordable housing has helped his populist credentials. But his track record has few notable policy achievements, said Bo, the political analyst.

For all his reformist leanings at university, Li shows the imprint of three decades within a hierarchical party structure.

"When Li speaks about housing matters, it's about giving directions and issuing directives. Li shows himself to be in command, well-versed and deeply briefed," said Beijing-based political analyst Russell Leigh Moses.

"At the same time, it's apparent that he expects Beijing's instructions to be carried out fully. In that respect, Li continues the new tradition of seeking to centralize political authority and policy-making in China."

'Half-match' bone marrow transplants wipe out sickle cell disease in selected patients

'Half-match' bone marrow transplants wipe out sickle cell disease in selected patients

Friday, September 21, 2012

In a preliminary clinical trial, investigators at Johns Hopkins have shown that even partially-matched bone marrow transplants can eliminate sickle cell disease in some patients, ridding them of painful and debilitating symptoms, and the need for a lifetime of pain medications and blood transfusions. The researchers say the use of such marrow could potentially help make bone marrow transplants accessible to a majority of sickle cell patients who need them.

After a median follow-up of two years, the transplants successfully eliminated sickle cell disease in 11 of 17 patients. Three were fully matched to their donors and eight received half-matched donor marrow. All 11 patients are free of painful sickle cell crises and 10 no longer have anemia. There were no deaths and no unexpected toxicities.

Six of the 11 patients (all half-matched) have stopped taking immunosuppressive drugs, although some still require narcotics for chronic pain because of sickle cell-related organ damage. Blood tests on the six patients show that their red cells are now completely derived from their donor's marrow.

Patients with severe sickle cell disease (SCD) face shortened life spans, intractable pain and eventual organ damage as a result of their disease, an inherited disorder caused by a mistake in the oxygen-carrying hemoglobin molecules in red blood cells. The flawed genetic code stiffens red cells, and shapes them into a pronged "sickle" that clump and stick into blood vessel walls, cutting off blood and oxygen to tissues and organs throughout the body.

SCD occurs in approximately one in 400 African Americans, and rarely in Caucasians. An estimated 100,000 people are currently living with sickle cell disease in the U.S.

Most patients die before age 50, and many suffer poor quality of life with frequent episodes of "off-the-charts" pain, and an increased risk for kidney failure, stroke, deep-vein thrombosis, and lung disease.

Treatments include blood transfusions and a drug, hydroxyurea. Many patients use narcotics to control severe pain and have repeat hospitalizations. Bone marrow transplants have been successful in curing some cases, but matching donors are rare and the procedure itself poses risk.

In the current study, 17 patients at the Johns Hopkins Hospital were offered bone marrow transplant options, including the use of half-matched donor marrow to try and replace their "sickled" blood cells with new, healthy ones. The transplants were successful in 11 of the patients, of whom eight were only half-matches. Results of the trial were published in the Sept. 6 early online edition of Blood.

"We're trying to reformat the blood system and give patients new blood cells to replace the diseased ones, much like you would replace a computer's circuitry with an entirely new hard drive," says Robert Brodsky, M.D., director of the Division of Hematology at Johns Hopkins and The Johns Hopkins Family Professor of Medicine and Oncology. "While bone marrow transplants have long been known to cure sickle cell disease, only a small percentage of patients have fully matched, eligible donors."

National registries often are of little help in finding donors for sickle cell patients, because most of those in need are African American and other minorities who are vastly underrepresented in registries, say the Johns Hopkins researchers.

To overcome the shortage of donors, investigators at Johns Hopkins developed techniques, recently tested in leukemia and lymphoma patients, to transplant with bone marrow that is half-identical or "haploidentical" to the patient's tissue type. Half-matched bone marrow can be obtained from parents, children and most siblings, and is extracted by needle from the hip bone.

For the study, the Johns Hopkins team screened 19 patients to find bone marrow donors with either half-identical or fully matched tissue. Each transplant candidate had experienced many severe pain crises, significant organ problems, or had failed hydroxyruea, the only drug known to curtail sickle cell symptoms. The team found donors for 17 of the 19 patients: 14 were half-identical and three were fully matched siblings. The youngest patient was 15; the oldest 46.

Before each transplant, sickle cell patients received a "conditioning" regimen of low-dose immunosuppression drugs, low toxicity chemotherapy, and low-dose total body irradiation. Brodsky says this gentler approach to pre-transplant therapy has made transplant possible for sickle cell patients whose tissues and organs have been ravaged by the disease.

After the transplant, all patients received high doses of the chemotherapy drug cyclophosphamide, which kills remaining blood cells, including diseased sickled cells, and preserves the donor's stem cells responsible for making new, healthy cells.

Of the 17 patients, six transplants were not successful; however, because of the reduced intensity of the conditioning regimen, all of these patients recovered their own blood cells.

There were no deaths, some infections, and only slight skin-related graft versus host disease symptoms in one patient, which cleared without therapy, the researchers reported. Some brain swelling occurred in three patients during the conditioning period and resolved without neurologic damage.

The Johns Hopkins doctors say that while the majority of patients in the trial had successful transplants, about less than half did not.

"Sickle cell disease patients undergo multiple blood transfusions throughout their lives and may have acquired antibodies against many different blood types, making it more difficult than usual to give patients donated bone marrow." says Javier Bola?os-Meade, M.D., associate professor of oncology at the Johns Hopkins Kimmel Cancer Center and principal investigator of the study.

Improving the rate of engraftment in haploidentical transplants for sickle cell disease remains a challenge, they say, but the researchers are looking for additional ways to overcome it, including increasing the number of stem cells transplanted and using other immunosuppressant drugs during the transplant.
