White: Newly identified stem cells may hold clues to colon cancer

Vanderbilt-Ingram Cancer Center researchers have identified a new population of intestinal stem cells that may hold clues to the origin of colorectal cancer.

This new stem cell population, reported March 30 in the journal Cell, appears to be relatively quiescent (inactive) ? in contrast to the recent discovery of intestinal stem cells that multiply rapidly ? and is marked by a protein, Lrig1, that may act as a "brake" on cell growth and proliferation.

The researchers have also developed a new and clinically relevant mouse model of colorectal cancer that investigators can now use to better understand where and how the disease arises, as well as for probing new therapeutic targets.

Colorectal cancer is the second leading cause of cancer deaths in the United States. These tumors are thought to arise from a series of mutations in intestinal stem cells, which are long-lived, self-renewing cells that gives rise to all cell types in the intestinal tract.

For more than 30 years, scientists believed that intestinal stem cells were primarily quiescent, proliferating only rarely in order to protect the tissue against cancer. Then, in 2007, researchers reported finding a population of intestinal stem cells (marked by the molecule Lgr5) that were highly proliferative.

Those findings "really changed the way we think about intestinal stem cells," said Robert Coffey, Jr., M.D., Ingram Professor of Cancer Research, co-chair of Vanderbilt's Epithelial Biology Center and senior author on the study.

"It came to so dominate the field that it raised the question about whether quiescent stem cells even exist?and that's where we enter into the picture."

Coffey's lab studies the epidermal growth factor (EGF) signaling pathway ? which includes a family of receptors known as ErbBs ? and its role in cancers of epithelial tissues, like the intestinal tract.

Postdoctoral fellow Anne Powell, Ph.D., led the recent experiments showing that Lrig1, a molecule that regulates ErbB activity, is present in intestinal cells that have the qualities of stem cells (self-renewal, and the ability to produce all the cells of the intestine).

"Essentially, what we show is that the Lrig1-expressing cells are stem cells and they are largely quiescent," Powell said. "We also show that they're distinct from the Lgr5-expressing stem cells that had become a sort of 'hallmark' stem cell population?with different gene expression profiles and different proliferative status."

They also showed that Lrig1 is not only a marker of intestinal stem cells, but also acts as a tumor suppressor and inhibits the growth and proliferative signals of the ErbB family ? acting as a sort of "brake" on cell proliferation that can lead to cancer.

Postdoctoral fellow Yang Wang, Ph.D., eliminated Lrig1 in mice and showed that nearly all of those mice developed intestinal tumors, providing further evidence suggesting that Lrig1 functions as a tumor suppressor.

The findings underscore the importance of ErbB signaling in the behavior of intestinal stem cells from which colorectal cancer may arise.

Most exciting, said Coffey, is that the mouse model his lab has generated as a part of these studies is one of the only mouse models to develop tumors in section of the intestines where most human tumors develop: the colon. One additional advantage of this model, in contrast to others, is that the tumors develop quickly and can be easily monitored with endoscopy, which will make it easier to assess how therapeutic interventions are working.

"We are fairly confident that this will be the 'go-to' model to study colon cancer in mice for the foreseeable future," Coffey said.

In one of the largest studies of its kind, a consortium of investigators from 13 countries led the University of Pennsylvania School of Nursing in the U.S. and the Catholic University of Leuven, Belgium in Europe, found that nurses who reported better working conditions in hospitals and less likelihood of leaving also had patients who were more satisfied with their hospital stay and rated their hospitals more highly. The study was released today in the current issue of the prestigious British Medical Journal.

The massive study, which in some countries involved every hospital, surveyed 61,168 bedside nurses and 131,318 patients in more than 1,000 hospitals in 13 countries over the course of three years, finding that in those hospitals with better work environments and fewer patients in each nurse's workload, patient and nurses both reported higher standards of care and more satisfied patients.

"Patients in European and U.S. hospitals with better work environments were more likely to rate their hospital highly and to recommend their hospital" to others, wrote the study's lead author, Linda H. Aiken, PhD, RN, a professor of nursing and sociology and director of the Center for Health Outcomes and Policy Research at the University of Pennsylvania School of Nursing.

Patient safety is also a concern in hospitals that have poor work environments and insufficient nurse staffing, said Walter Sermeus, professor at Catholic University of Leuven, Belgium, leader of the European consortium.

Nurses in Poland and Greece were three times more likely to give their hospitals a failing grade for safety than nurses in the U.S. and Norway. The majority of nurses in every country expressed a lack of confidence that hospital management would resolve problems in patient care.

Specifically the researchers found that:

• High nurse burnout and job dissatisfaction were common among hospital nurses in Europe and the U.S.
• On average, only 60 percent of patients were satisfied with their hospital care.
• Those nurses reporting high levels of burnout (notably in Greece and England) also reported an intention to leave their current positions.
• Each additional patient added to a nurse's workload increased the odds of a nurse reporting poor or fair quality of care.
• Patients were less satisfied with their hospital stay in those hospitals that had higher percentages of burnt out or dissatisfied nurses.

Policy implications for the findings suggest that despite the differences among the healthcare systems studied, particularly in terms of both organization and financing, all countries encountered problems of "hospital quality, safety, and nurse burnout and dissatisfaction." Many European nurses report they intend to leave their hospital positions, from 19 percent in The Netherlands to nearly half of all nurses (49 percent) in Finland and Greece, leading the researchers to ponder the potential for a worsening shortage of nurses.

A significantly lower proportion of nurses in the U.S. (14 percent) reported their intentions to leave their current positions, possibly due to increased efforts in the U.S. to improve hospital nurse staffing levels. Having fewer patients per nurse has been linked to better outcomes for patients, including lower rates of death following everyday surgeries. Nearly 7 percent or 400 in the hospitals in the U.S. have achieved "magnet status," so-called due to its ability to attract and retain nurses because of good work environments. No hospital in Europe has a similar "magnet" designation.

Alien invasion a threat to Antarctic ecosystem

SYDNEY (REUTERS) - In the pristine frozen continent of Antarctica scientists fear an alien invasion -- not from outer space, but carried in people's pockets and bags.

Seeds and plants accidentally brought to Antarctica by tourists and scientists may introduce alien plant species which could threaten the survival of native plants in the finely balanced ecosystem.

Invasive alien plants are amongst the most significant conservation threat to Antarctica, especially as climate change warms the ice continent, said a report in the Proceedings of the National Academy of Sciences Journal published on Tuesday.

More than 33,000 tourists and 7,000 scientists visit Antarctica each year by ship and aircraft, and a two month survey of visitors has found that many are carrying plant seeds picked up from other countries they have already visited.

The study vacuumed travelers' pockets, trouser and sleeve cuffs, shoes and inside their bags, and used tweezers to pry out accidentally hidden seeds. On average each person checked had just 9.5 seeds in clothing and equipment.

"The people that were carrying the most had lots and lots of seeds. They really were substantial threats," said Dana Bergstrom, from the Australian Antarctic Division.

"When we take things in through hitchhiking then we get species which are competitive. The plants and animals there are not necessarily competitive, so there's a good chance... we'd start losing various precious biodiversity on the (Antarctic) continent," Bergstrom told Reuters.

Amongst the alien species discovered were the Iceland Poppy, Tall Fescue Velvet grass and Annual Winter Grass -- all from cold climates and capable of growing in Antarctica.

The Antarctic Peninsula, where most tourists travel, is now considered a "hot spot" on the frozen continent and the warmer the climate, the easier for seeds to propagate.

"The peninsula is warming at some of the greatest rates on the planet," said Bergstrom.

The study, the first continent-wide assessment of invasive species in Antarctica, surveyed about 1,000 passengers during 2007-2008, the first year of the International Polar Year, an international effort to research the polar regions.

It has taken almost three years to identify the seed species and their effects on the icy continent.

Bergstrom said the one alien seed that had gained a foothold is Annual Winter Grass. It is a substantial weed in the sub-Antarctic and is on the Antarctic island of King George. It has also made its way to the tail part of the Antarctic continent.

"That's just one example of the weeds we picked up and a population of it has just been found in the last couple of seasons," she said.

Annual Winter Grass grows very well in disturbed areas like seal and penguin areas, and could propagate amongst the slow growing mosses around those colonies.

"If it got into those areas in the peninsula it would have the potential to overrun things," Bergstrom said.
